Cervical Proprioception Training
For dizziness, vertigo & headaches
If you’re experiencing dizziness, vertigo, or ongoing headaches, the cause isn’t always the inner ear, as can be the case for conditions such as vertigo/BPPV.
In fact, the neck plays a key role in how the brain interprets movement, position, and balance.
At Active Balance, we use a technique called cervical proprioception training, including laser headlamp therapy, to assess and retrain the connection between the neck, brain, and balance system to help relieve symptoms & improve quality of life.

What is cervical proprioception?
Cervical proprioception is the neck’s ability to sense its position and movement in space. Receptors in the upper cervical spine send constant feedback to the brain to help control balance, coordination, eye movements, and posture.
When these signals are impaired, a sensory mismatch occurs between the neck, inner ear, and visual system.
This often leads to:
- Dizziness or lightheadedness
- Vertigo or spinning sensations
- Cervicogenic (neck-related) headaches
- Poor balance or unsteadiness
- Neck pain and stiffness
- Difficulty with concentration or visual tracking
The Assessment Process
Our physios conduct a detailed assessment to determine whether your neck may be contributing to your symptoms. This includes:
- Laser-guided Joint Position Error Testing (JPET)
- Neck mobility, strength, and posture assessment
- Eye-head coordination and tracking tests
- Balance and postural control evaluation
- Differentiation between cervical and vestibular (inner ear) causes
Laser Headlamp Therapy
This simple yet highly effective tool involves wearing a lightweight headband with a laser pointer. You perform controlled head movements while aiming at targets on a wall. It helps:
- Retrain movement accuracy
- Improve neck-brain coordination
- Reduce dizziness and improve confidence
Who Can Benefit from Cervical Proprioception Training?
This treatment is often helpful for people experiencing:
- Persistent dizziness or vertigo, especially triggered by neck movements
- Neck-related headaches
- Symptoms following whiplash or neck trauma
- Ongoing neck stiffness with balance issues
- Combined neck and vestibular problems
- Post-concussion symptoms involving the neck
Our Treatment Approach
Your program is progressive and individually tailored. It may include:
- Head repositioning accuracy exercises
- Laser-guided coordination drills
- Deep neck flexor strengthening
- Eye-head and balance retraining
- Manual therapy for pain relief and mobility
We start with simple movements and progress to functional, real-life activities. Laser headlamps are available for loan to support you to practice at home between sessions.
Frequently Asked Questions
How long is a typical session?
Initial assessments usually take 45–60 minutes. Follow-up sessions are typically 30–45 minutes, depending on the treatment required.
How many sessions will I need?
This varies between individuals. Many people notice improvement within 4–6 sessions, while others benefit from a longer program. Your physiotherapist will provide a clear recommendation after the initial assessment.
Is this the same as vestibular rehabilitation?
Not exactly. Vestibular rehabilitation focuses primarily on the inner ear. Cervical proprioception training specifically targets neck-related issues. We often combine both approaches when needed for best results.
Do I need a referral?
No referral is required. You can book directly online or by calling us.
What should I wear or bring to the appointment?
Wear comfortable clothing that allows free neck and head movement. If you have any previous scans, medical reports, or a history of neck injury/whiplash, please bring them along.
Can I do the exercises at home?
Yes. You will receive a personalised home exercise program. We also loan laser headlamps so you can continue training effectively between clinic visits.
Is this treatment evidence-based?
Yes. Cervical proprioception retraining, including laser-guided exercises, is supported by research for managing neck-related dizziness, headaches, and balance issues.
Can this help after a concussion or whiplash?
Absolutely. Neck proprioception is often affected following whiplash and concussion. This treatment is frequently used as part of our broader Concussion Management programs.
What if I’m not sure whether my dizziness is from my neck or inner ear?
That’s exactly why we do a thorough assessment. We can differentiate between cervical and vestibular causes and recommend the most appropriate treatment plan (or a combination of both).
